How to claim Scholarship Funds

 

In order to receive your scholarship award, it is your responsibility to:

1Obtain Official Verification of Enrollment for academic year 2013-2014 which begins in the summer/fall of 2013.  As soon as you are enrolled and attending classes, obtain an Official Verification (or Certificate) of Enrollment form, usually available from your school’s Office of Admissions and Records, the Registrar, or the Office of Financial Aid.  (Note: Some schools may charge a small fee for this service.) This document will state that you are enrolled in the school, that you are attending classes, and that you are in good academic standing.  The school must affix its seal to this document. (A transcript or class registration form is not acceptable as an official verification of enrollment.) NO scholarship can be paid until the official Certificate/Verification has been received by SWCSRF.

Please note that you must be actively enrolled and attending classes as a fulltime student (ie. a minimum of 9 credits/semester toward a degree or certificate of graduation) in an accredited institution for a minimum of 2 semesters per school year in order to receive your full scholarship.

 2.      Mail Official Verification of Enrollment. Be sure the Official Verification/Certificate of Enrollment form is mailed directly to us at the following address:  SWCSRF,  P.O. BOX 3045, SAUSALITO, CA. 94966

 3.      Provide your correct mailing address, e-mail address and phone number while you are at school. Instruct us where you want the check sent and to whom — specifying your address at school or your parent’s/guardian’s address or the name and address of the school official who is to receive the check. The Scholarship Award check will be made out to you or to a school official, based on your instructions.

 4.      Collect your scholarship funds by March 14, 2014. You must claim your award by March 14, 2014  or the monies will be deemed forfeited and returned to the Sausalito Woman’s Club Scholarship Recognition Fund.

 

After we receive the Official Verification/Certificate of (fulltime) Enrollment and instructions on where to mail the check, our Treasurer will prepare a check and mail it to you.
